THREE BATS AERIAL — OPEN GYM WAIVER ADDENDUM This Open Gym Waiver Addendum (“Addendum”) supplements and forms part of the Three Bats Aerial Liability Waiver & Release Form previously signed by the participant. Participation in Open Gym sessions indicates full understanding and acceptance of the following additional terms and conditions. ELIGIBILITY - All Open Gym participants must be enrolled in adult level aerial classes. For any students under 18 years of age, a parent or guardian will be required to remain on the premises to supervise all activities. - A valid Emergency Contact Name and Phone Number must be provided prior to participation. ACKNOWLEDGMENT OF RISK Open Gym sessions are not instructor-supervised. Participants understand and agree that: - There will be no instructors on site during Open Gym hours. - Only moves, skills, and techniques that you are familiar with and confident performing safely without assistance or spotting may be practiced. - No new or unfamiliar moves may be attempted, including those seen on social media, YouTube, or other instructional sources. - Peer-to-peer spotting or teaching is strictly prohibited. - Only studio owners are authorized to touch, adjust, or modify rigging. - Participation is entirely at your own risk. USE OF PERSONAL EQUIPMENT Participants may bring and use their own apparatus or equipment during Open Gym under the following conditions: - All personal equipment must be in safe, working condition and approved by a studio owner before use. - Three Bats Aerial is not responsible for damage to or failure of personal equipment. - The participant assumes all risk associated with using personal apparatus or gear. EMERGENCY MEDICAL AUTHORIZATION In the event of a medical emergency, I authorize the Three Bats Aerial Studio Owners to call 911 and obtain emergency medical care on my behalf if deemed necessary for my safety and well-being. I understand that I am responsible for any costs incurred as a result of such care.
